2010 JSTU Banquet Registration Form

The cost of the banquet is $35 per person. You may bring your payment to the next meeting, or mail it to:

Notes:
(1) You do not need to be a member of Jersey Shore Trout Unlimited to attend our banquet.
(2) All banquet attendees must present government issued identification, either a passport or a driver's license with photograph, to Fort Monmouth security personnel when entering the Fort through the Rt. 35 entrance.
(3) We must submit a final list of names and other information (vehicle info, etc.) to Fort Monmouth security no later than March 19, so please submit your registration prior to that date.
